Tools and approaches for supporting transition planning will be outlined and some outcome data will be provided. This document will outline various options for young adults with developmental disabilities in the areas of work, education, and community support. A specific Canadian and British Columbian perspective will be included in the later sections of the review. Finally, recommendations for practice and for areas for future consideration will also be highlighted.

The Stacking the Deck program is an excellent evaluation tool designed to be used with any available game board to teach appropriate social skills to individuals with mental disabilities. Persons with disabilities can learn new skills, and increase existing ones while practicing appropriate social behaviours in a relaxed atmosphere. The program's social situations are derived from six areas of social behaviour: compliments, social interaction, politeness, criticism, social confrontation, and questions/answers.

For the child, play provides a prime medium for positive interaction with the surrounding world and the discovery of pleasure. However, participation in this formative activity by children with motor disabilities can be difficult. The Ludic Model introduces a method of occupational therapy for physically restricted children in which play is understood as a privileged medium through which children can be encouraged towards holistic development, while discovering pleasure through their own capacity to act. The model is applicable to a home or therapeutic setting and encourages optimal autonomy while facilitating a satisfying quality of life for the whole family. Abstract
This two-year study examined the challenges identified by urban Aboriginal families of
children with physical disabilities and determined practical solutions for policy makers
and service providers to help eliminate or overcome these obstacles. The study has
been coordinated by National Association of Friendship Centres, Ottawa and funded
through the Social Development Partnership Program – Office for Disability Issues
of the Government of Canada. Interviews were conducted with Aboriginal parents of
children with disabilities and service providers in the social and health services across
Canada. The study concentrated on Regina but site visits and interviews were conducted
in Montreal, Toronto and Vancouver. Telephone interviews were completed with
participants in Halifax. These site visits were very enlightening. Although, most of the
issues are common across the country, there are significant issues and unique concerns
in some locations.

This is our educational program for K-6th grade. The Know Your Buddy Book is a comprehensive resource guide designed to educate children about disabilities, illnesses and other medical challenges. Nurses, teachers and counselors use this resource guide to educate healthy children about other children with special needs. In addition, the program is a great tool to use when helping children who are returning to school after an extended illness.
