The transition planning process of various regions in the world will be explored which will include recommendations from the literature. Tools and approaches for supporting transition planning will be outlined and some outcome data will be provided. This document will outline various options for young adults with developmental disabilities in the areas of work, education, and community support. A specific Canadian and British Columbian perspective will be included in the later sections of the review. Finally, recommendations for practice and for areas for future consideration will also be highlighted.
